****Swoosh Membership & Insurance Information****
Swoosh Association Membership
Each league, team or association must become a member of Swoosh Basketball in order to qualify for any of the Swoosh Member Benefits. Single team membership cost is $30, Associations with two teams pay $50 annually, or multi-team membership cost is $100 annually. All Swoosh Memberships are in effect from September 1, 2008 (or when payment is received and verified) through August 31, 2009.
*****Sign up for Swoosh Insurance by September 1, 2008 and Association fees are waived*****
Player Insurance Programs
All of the accident insurance offered by Swoosh is secondary or excess coverage, meaning that medical benefits are paid in excess of the total benefits payable for the same loss under any other insurance or health plan. If there is no other coverage in effect at the time of the accident, the Swoosh plan becomes primary.
Following is a brief summary of the different types of coverage offered through Swoosh.
Individual Player Insurance
Individual Player Insurance is recommended for all teams that wish to participate in any Swoosh-sanctioned tournament and league. The cost is $10.00 per player and coverage is effective from September 1, 2008 to August 31, 2009 or when the Player Registration Form and payment are received, whichever is later.
A Player Registration Form (roster) must be completed and verified by the Swoosh National office for each team. The Individual Player Insurance offers $10,000 secondary accident insurance for any one injury, less a $100 deductible.
If a team is only going to play in Swoosh Leagues (not tournaments) their cost is $6.00 per player.
Liability Insurance Coverage
Any team, league, or association that is a current member of Swoosh and has registered all of their players through either the Individual Player Insurance Program is automatically enrolled and covered by Swoosh's liability insurance. This coverage applies to each Association as well as their Board of Directors, Officers, Administrators, Coaches and Participants, and offers up to $3,000,000 coverage.
Additional Insured Certificates
(Site Insurance)
Additional Insured Certificates are available for all teams that are current Association Members of Swoosh and have registered all of their players through one of the Player Insurance Programs. The cost is $20.00 per certificate or site. Associations that use more than one facility operated by the same entity may be able to save money by insuring the entire entity rather than each individual site. This applies to using several schools within the same district or more than one facility owned or operated by a city, county, or parks and recreation department. Please check with your local facility to make sure this meets their requirements.
A Site Liability Insurance request form should be completed to request Additional Insured Certificates.
Coaches & Officials Association
The Swoosh Coaches and Officials Association is a program that has been designed to protect individuals that coach or officiate by offering $10,000 secondary accident insurance for any one injury, subject to a $100 cash deductible, as well as up to $3,000,000 in liability coverage.
This coverage is unique in the fact that it is not only in effect for Swoosh events, but will also cover you for any sport that you coach or officiate (other than at the high school or college level).
The cost of this coverage is $20.00 annually and is effective from September 1, 2008 through August 31, 2009, or when payment and application are received, whichever is later. All coaches will be required to carry this coverage to sit on the sidelines during sanctioned tournaments.
